Please read this form carefully and be aware that by signing up and participating in Escape Plan’s self defense program, which also refers to its teachers, you will be releasing all claims of injuries rising out of this program that you or any other participants might sustain. The terms “I”, “me” and “my” also refer to parents or guardians as well as participants. In registering for these programs, you agree to the following:

As a participant in this program I agree to assume full risk of any damages or loss which I may sustain as a result of participating in any and all activities connected with or associated with such programs. I further recognize and acknowledge that all strenuous exertion or body contact poses a risk of serious bodily injury.

I agree to waive and relinquish any and all claims I may have as a result of participating in this program against Escape Plan, owners, instructors, hosting facilities or any person that might be directly liable for any injuries that might be sustained as a result of participating in this program.

I do hereby release and discharge Escape Plan and all other released parties from any and all claims for injuries, damage, or loss that I may have or that may accrue to me on account of participating in this program.

I further agree to indemnify, hold harmless, and defend Escape Plan and the other released parties from any and all claims of injury, damage, or losses sustained from anyone, and arising out of, connected with or in any way associated with my conduct and activities of this program. I understand the nature of the program for which I am registering, and have read and fully understand this waiver, release, and hold harmless agreement. I further understand that any advisement or warning of the particular risks of this program that I subsequently receive will be incorporated by reference into and become a part of this agreement.

On occasion, our staff may take pictures of participants in our programs, classes or events; or of people in the area. Please be aware that these pictures are for Escape Plan’s use, most likely on the web-site and for future promotional materials.
